where did fugazi get their name?

ian was reading a book about vietnam at the time, and the term came popping in a lot, so he liked it. it was used for a situation of extreme trouble.
he found out what it meant literally when they toured singapour or one of such countries, a fan told him it meant "fucked-up, got ambushed, zipped in". |

And then, jerry, as they were turning the pages, noticed one word standing out, glowing even, clear and bright, "Grateful Dead" and suddenly it was the only word that any of them could see, and so they all saw it as a sign.
