Did the ending/beginning of the film really happen? Or was it just in his book?

The ending/beginning was just far too surreal compared to the rest of the film, and she says something about how it was her secret to keep that he hated the end of his book, and she alluded that what happens in the end of the film happened at the end of the book.

None of you film jerks have seen this?
