For all the doubters, it is very real and known as the act of "nullification". I'm a nullo myself and the process is usually performed under more "sanitized" conditions. The performer in question wanted to make it seem as grotesque and painful as possible, hence the competion involved. I can only recommend it and would do it again if I could =)
