Twice.
Once was my wife's grandmother in a hospital room (very uncomfortable; I didn't know a lot of the family present and it was obviously a very emotional scene), and the second time was when I was behind a truck that hit a pedestrian. The truck had just started up from a full stop and was turning and hit a woman crossing the street, going less than 5 MPH. I didn't see the actual point of contact because I was immediately behind the truck; I just saw other people reacting to something. When I drove around the truck, I saw the woman lying in the street. A tiny trickle of blood came out of her nose, and suddenly became a river of blood running onto the street. I found out later that she had died.
