just the cd-r, like you. I do think one of my copies of this show is lossless though, so that's essentially the same as the original bootleg, minus the artwork. Actually, I have reproduced the artwork (at least somewhat) too.

it's stated that a "private video source" exists.

This audience-shot private video source has been synched-up with the soundboard audio from the bootleg and there is a dvd-r of this that also circulates. It looks like it was originally shot on a VHS camcorder and, some unknown generation of VHS later, transferred to digital. In other words, the picture quality isn't all that incredible, but it sounds good (except for a few brief drop-outs).
