I think I have seen one picture posted where these two were both present in the photograph.
Their mode of speech is a debased dialect found in outstate Minnesota, and shows the effects of Methamphetamine on the outlying region's population. This explains the similar tone and speaking style of their posts.

Though I like better the idea of Hayden and Betterthanthou being one and the same. Would explain how Better knew precisely how many times Hayden threw up and other intimate details, as well as why she's fascinated by such--and would explain why she thinks he's so funny (in other words, this is simply self-advertisement).
